The Reablement service provides short‑term care and support to adults within their own homes throughout the Halton area. The team focuses on service users’ strengths, enabling them to stay independent and improve their wellbeing. We aim to build service users’ confidence after a stay in hospital, illness or deterioration in health, preventing re‑admission to hospital or a premature move to a care home.
The Reablement Service operates 365 days per year. Care and support worker shift patterns are on a two‑week rota, working every other weekend. All posts include a mixture of early and late shifts. Early shifts commence at 7.30am and late shifts finish at 9pm or 11pm. Enhancements are paid for weekends and unsocial hours. You will receive a car allowance payment monthly in addition to your business mileage.

As a care and support worker you will create and maintain high‑standard care and support plans for service users, empowering them and promoting independence. Working within a multidisciplinary team, you will develop and maintain effective working relationships with a range of professionals, including therapy and social work teams. Candidates must possess excellent interpersonal skills, be able to communicate and listen effectively, and accurately maintain clear and concise written records and reports.
