An exciting and innovative opportunity has arisen within Keswick and Solway Integrated Care Community (ICC). We are seeking a Care Coordinator to join 'The Your Health Matters Team'.

The post holder will be based within the ICC and supported by the therapists in the well established 'Your Health Matters Team'. You will work closely with GP practices within the Primary Care Network (PCN) as well as ICC colleagues in this role. They will identify and work with a cohort of people to support their personalised care requirements using a range of interventions.

Training for the role will be provided.

Further training opportunities are available as Assistant Practitioner and/or Nurse Associate for interested candidates once established in the role.

The essential criteria for this role includes the requirement to have a care certificate or the willingness and ability to undertake this. For further information please see the attached Care Certificate document attached.
