Santé Partners zoekt een Leefondersteuner Verpleegkundige voor Wijk bij Duurstede in de thuiszorg. Je levert verpleegkundige zorg in de eigen woonomgeving, coördineert multidisciplinaire zorg en draagt bij aan continuïteit en kwaliteit van leven.
Je werkt in wisselende diensten tussen 7.00 en 23.00 uur, door de week en in het weekend, in Wijk bij Duurstede, Cothen en Langbroek. Een warme, collegiale werksfeer staat centraal en samenwerking met het wijkteam is essentieel.
