Advanced Practice Nurse (APRN) – Primary Care Geriatrics Clinic

• Practice medicine under physician supervision using sound medical judgment and adhering to all ethical considerations.
• Provide care for patients with acute and chronic illnesses encountered in older adults.
• Accountable for patient care and outcomes, seeking specialist consultation when needed.
• Engage with hospitalists when patients are hospitalized.
• Meet all clinical obligations of the medical practice, including accrediting agency compliance, CLIA, drug prescription compliance, HIPAA, State and Federal regulations, and safety regulations.
• Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence‑based medicine and practice guidelines.
• Follow all internal policies and procedures.
• Maintain complete and detailed records electronically in the EMR within 48 hours of each patient visit.
• Provide patients with information needed to promote health or prevent/manage disease or disability.
• Analyze and interpret patients’ histories, symptoms, physical findings or diagnostic information to develop appropriate diagnoses.
• Interpret diagnostic results and collaborate with supervising physician on appropriate actions.
• Exhibit compassion, quality, loyalty, and conscientiousness.
• Establish and maintain effective working relationships with patients, families, and team members.
• Document continuing education per licensure and certification requirements.
• Ensure patients see a physician every other visit.
• Maintain patient confidentiality in accordance with policy, procedure and HIPAA requirements.
• Enact proper chain of command for patient complaints.
• Perform additional duties as assigned.

• Graduate of an accredited school of nursing.
• Certified as a nurse practitioner with current state licensure.
• At least 1 year of professional nursing experience and 1–3 years as a primary care nurse practitioner.
• Prior experience with an electronic medical records system.
• Provide 5‑star service in accordance with Better Health Group’s mission.
• Skilled in traditional nursing disciplines of patient assessment and management.
• ACLS/BLS certification required.
• Demonstrate knowledge of appropriate patient care rationale.
• Handle emergency situations calmly and effectively.
• Ability to read, analyze, and interpret professional journals and regulatory documents.
• Effectively present information and respond to questions from diverse groups.
• Define problems, collect data, establish facts, and draw valid conclusions.
• Excellent written and verbal communication, teaching ability, and good telephone skills.
• Working knowledge of Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint).

• Full range of body motion, including handling and transferring patients, manual and finger dexterity, eye‑hand coordination.
• Standing, walking, pushing, bending, kneeling, and reaching overhead for prolonged periods.
• Lift and move patients and equipment; lift/carry up to 15 lbs.
• Corrected vision and hearing within normal range.
• Ability to sit for extended periods.
• Operate a motor vehicle and have own transportation.
