Position TitleAdvanced Practice Nurse (APRN) – Primary Care Geriatrics Clinic
Responsibilities- Practice medicine under physician supervision using sound medical judgment and adhering to all ethical considerations.
- Provide care for patients with acute and chronic illnesses encountered in older adults.
- Accountable for patient care and outcomes, seeking specialist consultation when needed.
- Engage with hospitalists when patients are hospitalized.
- Meet all clinical obligations of the medical practice, including accrediting agency compliance, CLIA, drug prescription compliance, HIPAA, State and Federal regulations, and safety regulations.
- Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence‑based medicine and practice guidelines.
- Follow all internal policies and procedures.
- Maintain complete and detailed records electronically in the EMR within 48 hours of each patient visit.
- Provide patients with information needed to promote health or prevent/manage disease or disability.
- Analyze and interpret patients’ histories, symptoms, physical findings or diagnostic information to develop appropriate diagnoses.
- Interpret diagnostic results and collaborate with supervising physician on appropriate actions.
- Exhibit compassion, quality, loyalty, and conscientiousness.
- Establish and maintain effective working relationships with patients, families, and team members.
- Document continuing education per licensure and certification requirements.
- Ensure patients see a physician every other visit.
- Maintain patient confidentiality in accordance with policy, procedure and HIPAA requirements.
- Enact proper chain of command for patient complaints.
- Perform additional duties as assigned.
- Graduate of an accredited school of nursing.
- Certified as a nurse practitioner with current state licensure.
- At least 1 year of professional nursing experience and 1–3 years as a primary care nurse practitioner.
- Prior experience with an electronic medical records system.
- Provide 5‑star service in accordance with Better Health Group’s mission.
- Skilled in traditional nursing disciplines of patient assessment and management.
- ACLS/BLS certification required.
- Demonstrate knowledge of appropriate patient care rationale.
- Handle emergency situations calmly and effectively.
- Ability to read, analyze, and interpret professional journals and regulatory documents.
- Effectively present information and respond to questions from diverse groups.
- Define problems, collect data, establish facts, and draw valid conclusions.
- Excellent written and verbal communication, teaching ability, and good telephone skills.
- Working knowledge of Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint).
- Full range of body motion, including handling and transferring patients, manual and finger dexterity, eye‑hand coordination.
- Standing, walking, pushing, bending, kneeling, and reaching overhead for prolonged periods.
- Lift and move patients and equipment; lift/carry up to 15 lbs.
- Corrected vision and hearing within normal range.
- Ability to sit for extended periods.
- Operate a motor vehicle and have own transportation.
