Overview

Job Summary:

The ambulatory clinical pharmacist provides safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. Embedded within a multidisciplinary clinical team and under a collaborative practice agreement with physicians, the pharmacist delivers direct patient care via in-person or telephone interventions. The pharmacist consults with patients and physicians to identify, resolve, and prevent medication-related problems and to establish mutually agreed-upon treatment goals.

The pharmacist assesses medications for indication, effectiveness, safety, tolerability, and adherence, and titrates/manages/monitors therapy to optimize clinical outcomes while addressing gaps in care.

Responsibilities
  • Assumes responsibility for delivering high-quality medication disease management services for the assigned patient care area.
  • Demonstrates proficiency in medication and disease management, and in-depth knowledge of medications, clinical conditions, and current treatment guidelines for the assigned clinical areas.
  • Gathers appropriate clinical data, imaging, and laboratory information to inform management decisions of medication therapy and related diseases and conditions.
  • Assesses the patient’s medication regimen for appropriateness, effectiveness, tolerability, safety, and potential adverse events.
  • Makes clinically appropriate changes to regimens to maximize effectiveness and patient satisfaction, while minimizing acute and chronic complications. Provides in-services, comprehensive drug information, and educational materials to members of the medical, nursing, and other healthcare professionals.
  • Provides patients with appropriate educational materials to help them understand and actively participate in their care.
  • Documents encounters in the electronic health record and communicates information to the referring physician and next provider of care as appropriate.
  • Attends, participates in, and, when requested, presents at department educational programs.
  • Teaches and trains pharmacy interns, residents, students, technicians, and new pharmacists in relevant areas of practice, policies, and procedures.
  • Supports ambulatory clinical pharmacy leadership in implementing new clinical programs and sites, including program setup, staff education, and certification; coordinates and supervises work activities and resources, ensuring services and work meet departmental standards and regulatory requirements and are billed appropriately.
  • Advises and supervises clinic assistants and pharmacy interns; assumes final responsibility for the accuracy of their work.
  • collaborates with facility staff, medical and nursing teams, support staff, and administration to integrate clinic services into site workflows and demonstrate the value of the ambulatory clinical pharmacy program.
  • Assists with review and revision of departmental policies and procedures as requested.
  • Applies policies to complex situations and identifies areas for policy development or improvement.
Qualifications
  • Education:

    Bachelor’s Degree-Pharmacy (Required).
  • Experience:

    Minimum of 2 years clinical experience (Required).
  • Licensure/Certification:
    Licensed Pharmacist (default issuing body) and PA license within 90 days (State of Pennsylvania).
