The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner to 'float' to various skilled nursing facilities in northeast Pennsylvania.
Job DutiesThis position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner (AP), prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The AP provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re‑admissions to the acute care setting.
This position has a $10,400 float differential and will travel to these locations, but are not limited to:
- Mountain Top Rehabilitation & Healthcare Center, Mountain Top
- Birchwood Rehabilitation and Healthcare Center, Nanticoke
- Lakewood Rehabilitation & Healthcare Center, Nanticoke
- The Meadows, Dallas
- Embassy of Wyoming Valley, Wilkes Barre
- Embassy of Scranton, Scranton
- Oak Ridge Rehabilitation & Healthcare Center, Scranton
Geisinger has much to offer:

- Attractive schedule - Monday - Friday 8 am – 4:30 pm
- Rotating call schedule
- No weekend hours
- Continuing Medical Education – 10 days and $3,000 annually
- Six paid holidays
- Paid Time Off - 23 days
Responsibilities:
- Review reports
- Communicate with SNF staff for patient needs
- Treat patients for acute needs, admissions, discharges, etc.
- Manage extremely complex patients, see every day or at least once a week
- Approximately 12 patients a day
- Length of time per patient and number of patients is per need
Master's Degree
