  • At least 2 years of LPN work experience is required.
  • A typical schedule includes Monday through Friday 9 am to 4:30 pm or 8:30 am to 5 pm
  • You'll provide infusion services in the GHIS Ambulatory Infusion Suite, including IV access placement, independent administration and monitoring of infusion therapies, patient scheduling coordination, and support of RN clinical operations.

Position Details

The Licensed Practical Nurse Navigator is a licensed nurse who guides defined patient populations through complex care pathways over time. Focuses on access, referral accuracy, sequencing of care, and continuity across services and settings, rather than daily clinic operations. Accountable for improving patient access, care coordination, and referral completion through timely, high‑quality navigation activities. Nurse will focus on patient journey over time and not individual clinic visits;

referral triage and access management; coordination across specialties, departments and locations; and ensuring patients care gaps are resolved. Performance is evaluated using defined Key Performance Indicators (KPIs).

  • Utilizes clinical knowledge to determine actions necessary to facilitate patient introduction to the program.
  • Reviews and triages incoming referrals for clinical appropriateness and urgency and ensures required records, diagnostics, and prerequisites are obtained prior to visits
  • Effectively collaborates with other healthcare providers in coordinating and implementing the patients plan of care, to include evaluation, treatment, and surveillance.
  • Effectively communicates with patients via mail, telephone, and in person regarding information pertinent to their care.
  • Coordinates patient needs for the program.
  • Serves as the point of contact for providers.
  • Coordinates patient activities, such as support groups, educational series, as appropriate.
  • Accountable for supporting and promoting internal referral retention and capture to support organizational goals.
  • Provides limited patient and family education based on individual needs either in person or by telephone.
  • Maintains systems and processes sufficient to assure timely follow-up and surveillance of patients.
  • Serves as a liaison with Scheduling Services to ensure appropriate scheduling of patients.
  • Develops and implements patient screening tools and efficient clinic procedures.
  • Assists in the development and implementation of efficient patient navigational procedures.
  • Investigate patient concerns including appointment access, communication, and billing.
  • Responsible for conducting medical record reviews to report findings to physician specialists.
  • Tracks patient status from referral through completion of care. Monitors follow-up, surveillance, and completion of care pathways
  • Helps patients navigate system barriers including access, scheduling and communication issues.
