Key Responsibilities:

• Assess and implement individualized care plans for residents

• Liaise with healthcare professionals and families for effective care

• Administer medications and monitor residents' conditions

• Support new staff orientation and care team development

• Participate in care plan reviews and interdisciplinary meetings

Requirements:

• Graduate of an accredited Practical Nursing program

• Current BCCNM license as an LPN

• Valid Occupational First Aid Level 1 certification

• Previous long-term care experience preferred

• Effective communication and computer skills
