This full-time role involves reporting to the Director of Care, where you will lead in implementing individualized care plans for residents. With an emphasis on collaboration, you’ll engage with various stakeholders to ensure effective healthcare delivery and uphold residents’ rights.

Key Responsibilities:

• Conduct assessments and evaluate residents’ healthcare needs

• Monitor changes in residents’ physical and psychological conditions

• Work collaboratively with healthcare providers for quality care

• Provide education and coaching to care staff

• Participate actively in care conferences and plan reviews

Requirements:

• Completion of an accredited Practical Nursing program

• Current BCCNM license to practice as an LPN

• First Aid Level 1 certification required

• Previous long-term care facility experience preferred

• Must pass a Criminal Record Check as per provincial guidelines
