Overview
Sentara Regional Resource Pool located in Hampton Roads is hiring an Inpatient Case Manager, MSW. Days/PRN for the Resource Pool. Required to work 4 hospitals in the Southside or Western Tidewater regions. Location and unit assigned based on greatest need. There are three region options. Must be within 75 miles and 90 minutes of all four hospitals in assigned region.
Responsibilities- Provide medical social work services to patients in the hospital setting, assess social, psychological, cultural, environmental, and financial situations, and disposition needs.
- Collaborate with patients, families, healthcare team members, and community organizations; use advocacy to identify needs and resources in hospital and community.
- Develop and evaluate transition plans considering diagnosis, prognosis, treatment course, past and present services, goals, provider options, and available benefits.
- Work across designated hospitals/units within the case management specialty as assigned by region.
Operational hours: 8:00 AM – 4:30 PM, 7 days per week. Flex hours: must preschedule at least 48 hours per calendar month; 16 of these hours should include weekend availability. Regions:
Southside locations (Norfolk General, Leigh, Princess Anne, VA Beach General);
Peninsula locations (Obici, Careplex, Williamsburg Regional, Albemarle Medical Center);
Western Tidewater (combination of 2 Southside and 2 Peninsula facilities: SNGH, SLH, SPAH, SVBGH, SOH, SCH, SWRMC, SAMC).
- Education:
Master’s degree in Social Work. - Certification/Licensure:
Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility; BLS required within 90 days of hire.
- One year of related experience.
- Must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
