Outpatient Therapist – The Pavilion at Williamsburg Place – Hampton, Virginia

Purpose Statement: The Therapist I is a member of a multidisciplinary team responsible for providing direct clinical services to patients and their families, with an emphasis on completing Psychosocial Assessments; providing individual, group, and family therapy; facilitating psycho‑educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and linking patients to outside resources.

• Conduct psychosocial assessments and develop initial discharge plans.
• Develop and implement treatment plans based on assessment, coordinating additional services as needed and revising as necessary.
• Conduct individual, group, or family sessions in alignment with the patient’s treatment plan.
• Apply psychotherapeutic techniques and interventions to treat diagnosed emotional and behavioral disorders.
• Participate in multidisciplinary treatment planning conferences, present patient information, and recommend effective interventions.
• Maintain an assigned caseload.
• Develop and implement special patient groups or program modules.
• Prepare progress notes and treatment updates in a timely manner and maintain all clinical documentation per regulatory and accrediting standards.
• Ensure each patient has a structured and appropriate post‑hospitalization care plan; act as liaison to referral sources and support continuity of care.
• Provide crisis intervention to patients and families in acute distress.
• Introduce and coordinate a wide range of community resources (financial, housing, legal, social/recreational, occupational, spiritual, support groups).
• Perform case management, discharge or after‑care planning as needed.

• Master’s Degree in Social Work, Counseling, or Psychology is required.
• At least one year of experience as a therapist with all age groups.
• Clinical interviewing and assessment using the Diagnostic Statistical Manual of Mental Disorders.
• Ability to assess, plan, and implement therapeutic social work services in a multidisciplinary setting.
• Proficient assessment skills, extensive knowledge of social work principles and practice, and familiarity with governmental eligibility programs to mobilize and coordinate resources effectively.

• Requisite state or national licensure for professional therapy providers or eligibility for state licensure as a professional therapy provider.
• CPR and de‑escalation and restraint certification required (training available upon hire and provided by facility).
• First aid may be required based on state or facility requirements.
