Wellness Nurse (LPN)Role Overview
As a Wellness Nurse (LPN), you’ll play a vital role in delivering high-quality care to our residents. You will administer medications, support daily wellness routines, supervise care staff, and help ensure each resident’s health and dignity are prioritized.
Key Responsibilities- Provide direct nursing care and supervise medication care specialists.
- Administer medications, treatments, and procedures per physician’s orders.
- Document resident care and update care plans as needed.
- Monitor for changes in residents’ health and communicate updates to physicians and families.
- Assist with resident move‑ins and orientation.
- Provide leadership and guidance to care staff, including performance evaluations and coaching.
- Participate in emergency response procedures and ensure safety standards are upheld.
- Maintain accurate resident records and ensure compliance with community policies and state regulations.
- Promote resident independence, wellness, and participation in daily activities.
- Foster a positive, respectful environment for residents, families, and team members.
- Active LPN license in the state of employment.
- Minimum 1 year of nursing experience; experience with Alzheimer’s/dementia preferred.
- Strong communication, interpersonal, and leadership skills.
- Compassionate, patient‑centered approach to care.
- Strong observational skills and attention to detail.
- Annual flu vaccination required (medical/religious exemptions accepted).
