The University of Vermont Medical Center is seeking a Home Infusion Nurse Coordinator (In Patient Home) to ensure smooth transition of patients to home infusion, educate patients and caregivers, and coordinate with the care team to optimize outcomes across the continuum of care.
Responsibilities include overseeing infusion procedures, vascular access, safety recommendations to the MD, and maintaining quality data while supporting quality improvement initiatives within the home infusion program.
