Currently seeking a full-time Physical Therapist for a position that is available in a skilled nursing facility. Company has a strong team environment and new graduates are encouraged to apply. Physical Therapists are responsible for: the assessment and treatment of patients;

completing documentation; planning and developing new programs; communicating with patients, patients’ families and physicians and staff; presenting in-services; and assisting in general operation of the department.

Key Responsibilities:
  • Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional ability.
  • Develop and implement personalized physical therapy treatment plans based on clinical findings and physician orders.
  • Provide therapeutic interventions including exercise, gait training, manual therapy, pain management, and use of modalities.
  • Document all evaluations, treatments, and resident progress in accordance with regulatory, facility, and professional standards.
  • Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the healthcare team to ensure coordinated care.
  • Comply with all federal, state, and facility policies including HIPAA, Medicare/Medicaid documentation guidelines, and infection control protocols.
Requirements:
  • Must be a graduate of a accredited therapy program (Bachelor Degree required. Masters Degree Preferred)
  • Must possess or be eligible for state licensure
  • Some previous inpatient rehab or LTC/SNF experience preferred but not required.
