We are seeking a dedicated physician to provide primary care rounding at post-acute facilities in the Wilmington, DE area. This is a full-time role with schedule flexibility, allowing you to build a rewarding practice while maintaining work-life balance.

Position Details:

  Wilmington, DE

• Setting:
  Skilled Nursing Facilities (SNF) and Assisted Living Facility (ALF)

• Schedule:
  
  Full-time, 5 days per week

• Rounding flexibility:
  Set your own schedule during daytime hours

Responsibilities:

• Conduct primary care rounding in SNFs and an ALF

• Manage chronic diseases and acute medical conditions

• Develop and implement care plans

• Collaborate with interdisciplinary teams and facility staff

• Provide mentorship and oversight to nurse practitioners

• Engage with patients, families, and facility leadership to ensure quality care

• Participate in care coordination and network quality initiatives

Qualifications:

• MD or DO with board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics

• Active Delaware medical license (or ability to obtain)

• Experience in post-acute care, geriatrics, or chronic disease management is a plus

• Strong communication and collaboration skills
