About the role
- We are seeking a dedicated Long-Term Care (LTC) Pharmacist to join our fast-growing LTC pharmacy team. As an LTC Pharmacist, you will play a critical role in ensuring safe, accurate, and timely medication management for residents in LTC Facilities
What you'll do
Medication Verification & Clinical Review- Perform final verification of prescriptions, ensuring accuracy in drug, dose, directions, and day supply.
- Conduct DURs (Drug Utilization Reviews), allergy checks, interactions, and therapeutic duplications.
- Review MARs, facility orders, and provider instructions for clarity and compliance.
- Provide clinical recommendations to providers and facilities.
- Review PRN and routine medications for appropriateness.
- Participate in care-plan reviews or medication audits as needed.
- Assist in Consulting Pharmacist duties related to Medication Regimen Reviews
- Ensure compliance with LTC regulations (Medicare/Medicaid, CMS, DEA, State Board of Pharmacy).
- Maintain accurate records for CII-CV medications, perpetuals, and emergency kits.
- Oversee return-to-stock, expired medications, and documentation required for audits.
Qualifications
- Doctor of Pharmacy (Pharm
D) or Bachelor of Pharmacy (BSPharm). - Licensed in State of Delaware
- Strong knowledge of LTC pharmacy regulations (Medicare/Medicaid billing)
- Excellent communication skills with nurses, providers, and internal teams.
- Ability to manage fast-paced workflow with attention to detail and accuracy.
- Comfortable using pharmacy management systems).
