Case Manager (RN) - Outpatient - Primary Care
Christiana Care Primary Care Practice;
Up to 4 days per week on-site required (depending on the practice). Monday to Friday, 8:00 am–4:30 pm with on‑call duties as scheduled.
This role involves coordinating care for a complex patient population with medical and social determinants of health needs. The Case Manager works to improve outcomes and reduce preventable readmissions by coordinating the utilization of healthcare resources, planning transitions, and facilitating the attainment of quality, clinical, financial, and member satisfaction goals.
PrincipalDuties & Responsibilities
- Perform ongoing telephonic case management activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of case‑managed members.
- Collaborate with patients, caregivers, and multiprofessional care team members to develop a member‑centered plan of care that meets identified goals and outcomes.
- Develop, implement, evaluate, and revise case‑management care plans in accordance with eligibility criteria, contractual guidelines, and members’ physical and psychological needs throughout the continuum of care.
- Identify system barriers to care and participate in strategies to remove barriers through a coordinated problem‑solving process.
- Promote member wellness and autonomy through advocacy, communication, education, and referral to community resources or other case‑management programs such as disease‑management programs.
- Identify appropriate providers and facilities across the continuum of services and ensure timely, cost‑effective use of resources.
- Collaborate with transitional, embedded, and inpatient case managers in discharge and transitional care, coordinating resources for members across the continuum.
- Stratify and/or validate member risk level during each transition and interaction.
- Work with health‑care providers in settings such as PCP offices, hospitals, skilled nursing facilities, and home‑care agencies to facilitate safe transitions and comprehensive communication of the treatment plan to the multidisciplinary team.
- Registered Nurse with a BSN or relevant experience in Utilization Management or Case Management.
- DE RN License or Compact Licensure; ability to obtain a non‑compact state RN licence as needed.
- Basic Life Support (BLS) certification required.
- Case‑management certification preferred through the Commission for Case Management (CCM) within 18 months of eligibility.
- Advanced certification recommended.
Hourly Pay Range: $41.28–$66.05 /hr.
This pay rate/range represents Christiana Care’s good‑faith estimate of compensation at the time of posting. The actual salary offered to a successful candidate will depend on individual factors, including skills, experience, and qualifications relevant to the role.
