Case Manager

Are you passionate about making a difference in the lives of those who have served our country? We are seeking a compassionate, resourceful, and driven Case Manager to support veterans and their families through housing crises and help them achieve long-term stability.

Schedule:

Monday-Friday, 8:30am-4:30pm
Pay Rate: $24/hour

• Make a direct impact in the lives of veterans and their families by helping them secure stable housing and access critical resources.
• Manage a meaningful caseload where you'll build strong relationships and see the results of your work firsthand.
• Work within a collaborative, supportive team focused on veteran advocacy, housing stability, and long-term success.
• Utilize a Housing First approach and trauma-informed care practices to create lasting positive outcomes for veteran households.

• Manage a caseload of veteran households and provide ongoing case management services.
• Assess eligibility, identify service needs, and develop individualized housing stability plans.
• Connect veterans and families to housing resources, VA benefits, public assistance programs, and community services.
• Coordinate services with internal teams and community partners to achieve successful outcomes.
• Maintain accurate, compliant case documentation and ensure adherence to program regulations and agency policies.
• Conduct outreach activities and cultivate referral partnerships within the community.
• Assist eligible households with financial assistance programs and resource navigation.
• Participate in program, community, and stakeholder meetings as needed.

• Bachelor's degree or equivalent experience in Social Services, Human Services, Nonprofit Management, Case Management, or a related field.
• Minimum of two (2) years of experience in social services, nonprofit programs, case management, or related human services work.
• Experience working with diverse populations and low-income households; veteran services experience is highly valued.
• Strong organizational, communication, time management, and interpersonal skills.
• Proficiency with Microsoft Office applications and case management databases.
• Valid driver's license, reliable transportation, current vehicle registration, and automobile insurance.

• Comfortable building relationships with clients, community partners, landlords, and stakeholders.
• Able to maintain professionalism, confidentiality, and composure in challenging situations.
