Location: Bourbonnais, Illinois and Wilmington, Illinois

Weekend-based, requires travel between Bourbonnais and Wilmington facilities.

Altea Healthcare is looking for a Certified Nurse Practitioner (NP) to join our post-acute and skilled nursing care team. Whether you are an experienced provider or a recent graduate, we offer training, mentorship, and support to help you succeed.

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

• Perform physical exams and review medical histories.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

• NP License (or eligibility to obtain)
• All experience levels welcome – training and mentorship available.
• Passion for geriatrics, internal medicine, or primary care.
• Strong team player with excellent communication skills.
