Position: Licensed Practical Nurse (LPN)

Licensed Practical Nurse (LPN) – Harmony Park at Wilson

Provide direct nursing care to residents, supervise the day‑to‑day nursing activities of CNAs/GNAs and other nursing personnel, and ensure that the highest quality care is maintained at all times in accordance with federal, state, and local standards.

Duties and Responsibilities Administrative Functions
  • Direct the day‑to‑day functions of the CNAs/GNAs or other nursing personnel in accordance with current rules and regulations.
  • Ensure that all assigned nursing personnel comply with the written policies and procedures established by this Facility.
  • Periodically review policies, procedure manuals, job descriptions, etc., and make recommendations for revisions.
  • Provide discipline and performance evaluations of assigned CNAs/GNAs and other nursing personnel.
  • Issue verbal and written disciplinary warnings for violations of Facility rules.
  • Meet with nursing personnel to plan shift services, programs, and activities.
  • Maintain current Nursing Service Procedures Manual and ensure its compliance.
  • Cooperate with other resident services to coordinate nursing services.
  • Participate in the Facility’s quality assurance program and in government agency surveys or inspections.
  • Review and update resident discharge plans and assist in their development.
  • Admit, transfer, and discharge residents as required; complete accident and incident reports.
  • Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
Charting and Documentation
  • Complete and file required record‑keeping forms or charts upon resident admission, transfer, and/or discharge.
  • Encourage physicians to review treatment plans and record orders.
  • Receive telephone orders and record on the Physician’s Order Form.
  • Transcribe physician’s orders to resident charts and treatment plans.
  • Chart nurses’ notes in a descriptive manner reflecting care delivered and resident response.
  • Submit accident and incident reports to the Director of Nursing.
  • Maintain accurate records of diet orders and transfer forms.
  • Ensure documentation of unauthorized discharges.
  • Sign and date all entries in the resident’s medical record.
Drug Administration Function
  • Prepare and administer medications as ordered by the physician.
  • Verify resident identity before administering medication.
  • Maintain adequate floor stock and report needs to staff.
  • Order prescribed medications and supplies following established policies.
  • Accurate narcotic counts before and after each shift.
  • Notify supervisor of all drug and narcotic discrepancies.
  • Review medication cards for completeness and adherence to stop order policies.
  • Notify the attending physician of automatic stop orders before last dosage.
  • Dispose of drugs and narcotics in accordance with established procedures.
Personnel Functions
  • Inform supervisor of staffing needs when personnel fail to report for work.
  • Provide feedback and recommendations for performance evaluations.
  • Develop work assignments and reassign personnel based on skill and qualification.
  • Lead, educate, counsel, and discipline nursing personnel as appropriate.
  • Make daily rounds to ensure all assignments are performed to standard.
  • Document disciplinary issues and report to supervisor.
  • Assist in resolving personnel disputes and reassign or remove staff if needed.
  • Maintain positive rapport with inter‑departmental personnel.
  • Receive and provide nursing reports upon shift changes.
  • Report occupational exposures to blood, body fluids, and hazardous chemicals.
Nursing Care Functions
  • Inform nursing personnel of new admissions and room assignments.
  • Ensure rooms are ready for new admissions and greet residents.
  • Participate in orientation of new residents and families.
  • Consult with physician and review resident charts for treatments, medication orders, and diets.
  • Maintain nursing objectives and standards.
  • Perform direct nursing duties such as catheterization, feeding, dressing changes, and medication administration.
  • Apply restraints when necessary and in compliance with policies.
  • Use automated external defibrillator as required.
  • Perform CPR and monitor seriously ill residents.
