• Conduct direct education for CKD participants virtually as part of the Kidney Smart program.
• Provide telephonic call support to CKD participants in focused areas across the country, including education, scheduling, rescheduling, and targeted Spanish support.
• Review, educate, and answer questions on all modality options for dialysis, including transplant, hemodialysis, peritoneal dialysis, and home hemodialysis.
• Provide local coverage support with opportunities and data entry as needed; complete data entry and tasking into the data management system.
• Perform other duties as assigned.
