Hours: Every Saturday & Sunday. Weekend option is a salaried position paid for 40 hours/weekend with on-call requirement.

Orientation & Training: Must be available for orientation and training M-F 8-5 for the first 90 days.

The Home Health and Hospice Nurse, RN, Baylor is responsible for working from 5pm Friday thru 8am Monday and taking call as assigned during these hours administering skilled nursing care to patients requiring intermittent, professional nursing services to include Home Health and Hospice patients.

• Registered Nursecurrentlylicensedtopracticenursinginthestateof North Carolina . Minimum of one-year experience as a professional nurse in an acute care setting.
• Maintain current
  
  CPR certification(BCLS)
• Previous experience working with terminally ill patients preferred
• Basic computer skills/knowledge and experience in electronic medical records
• Ability to understand and communicate well with patients, family, staff, physicians and the public
• Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
• Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient’s individualized a plan of care
• Possess valid Driver's License and reliable transportation

• Conducts the admission of the patient to the Agency by: obtaining a medical history, to include Oasis when needed, particularly as it relates to the present condition, from the patient and / or family member(s);performingaphysicalassessmentofthepatient;assessing the patient'ssupport systems to determine what assistance will be available from family members and their ability to meet the patient’s immediate needs upon admission and throughout care; assessing the patient, family member(s) andhomesituation to determinewhathealthteachingwill be required;assessing
  
  the patient’s condition and home situation to determine if other home health services are needed; interpretingnursingandotherservicesofthe
  
  Agencytopatient'sandfamiliesasapartofplanning for care.
• Developsandimplementsthepatientcareplanincooperationwithother Agencyprofessionalsand paraprofessionalswhowillbeprovidingcaretothepatientin Matrix Care.
• Managesassignedcasesandprovidesskillednursingcareasoutlinedonthephysician'splanoftreatmentandinaccordancewiththe
  
  Agency'spoliciesandprocedures.
• Observes signs and symptoms and reports to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patient’s physical or emotional conditionandoutcomesofintervention.
  
  Ensureson-goingevaluationofpatient/family response to care.
• Adequately educates, teaches, supervises, consults, and counsels the patient / family members regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern.
  
  Alsoprovidesspecializedhospicetrainingtootherstaff,familymembersandinformal caregiverstoensureadequate care. careand safety.
