Coastal Horizons Center is seeking a Dedicated and Qualified TASC Care Manager to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability For Safer Communities) Program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services.

Primary duties include administering psycho-social screenings/assessments, coordinating linkage to behavioral health treatment as well as medical, educational, and vocational services, and providing ongoing professional care management services for the justice-involved population.

Education:

Bachelor’s degree in criminal justice or a related human services field, or an associate degree with four years of experience in justice or human services. Exception to

Educational Requirements:

North Carolina Peer Support Specialist certification and a minimum of 2 years of experience in justice or human services.

• Strong communication, organizational, and time-management skills.
• Ability to work independently and manage a caseload effectively.
• Proficiency in using electronic health record systems and remote collaboration tools.
• Demonstrated ability to collaborate with diverse stakeholders.

$42,000 - $45,000 commensurate with experience, plus medical/dental, life, disability, 401K, and paid time off.
