Position Information

  LPN (4 on 2 off ) DAY SHIFT 6a - 2p.

Key Responsibilities

Nursing Care (85%):

• Direct day-to-day functions of nursing assistants per facility guidelines.
• Conduct admissions, transfers, and discharges of residents.
• Implement and update resident care plans.
• Administer medications and treatments per physician orders.
• Ensure accurate documentation in electronic medical records (Point Click Care).

Administrative Support (10%):

• Complete required medical forms, reports, and evaluations.
• Assist in training staff and maintaining data resources.

Other Duties (5%):

• Participate in staff meetings and additional projects as needed.

Minimum Qualifications

• Education:
  
  LPN degree from an accredited institution.
• Experience:
  
  1 year in a similar nursing role.
• Licensure:
  Valid LPN license in Oregon.

Preferred Qualifications

• Experience with electronic medical records systems (PCC).
• Strong communication skills and a patient-focused attitude.

Physical Requirements

• Ability to handle the physical and emotional demands of the job.
• Clear verbal communication and manual dexterity for typing and writing.
