Job Introduction

We have an exciting opportunity for a Band 6 Registered Nurse/ Paramedic to join our Hospital at Home (H@H) Service across Bath and North East Somerset, Swindon, and Wiltshire (BSW).

This is an excellent time to join the service as it undergoes a period of transformation and development, creating opportunities to contribute to innovative models of care and service improvement.

Working across a variety of community settings and patients' homes, you will play a key role in delivering high-quality acute care outside of traditional hospital environments.

As part of a multidisciplinary team, you will work collaboratively across acute, community, primary care, and social care services to provide assessment, treatment, and ongoing clinical management for patients requiring hospital-level care  service supports both admission avoidance (step-up) and early supported discharge (step-down) pathways, ensuring patients receive timely care in the most appropriate setting.
