This is an exciting opportunity for a passionate and patient focused Nurse to play a key role in delivering high quality care beyond the traditional hospital setting.
As a Specialist Nurse
, you’ll support patients in their own homes, helping to facilitate early discharge from hospital and prevent unnecessary admissions, while delivering safe, effective, and compassionate care.
You’ll work as part of a supportive multi disciplinary team, making a real difference to patient outcomes and experiences every day.
This role will see you working 37.5 hours per week over 5 days (including occasional weekends) and will cover the South West London and surrounding area so being confident on the road is a must.
