Merton’s Family Wellbeing Service are looking for a Family Wellbeing Case practitioner who is passionate about improving outcomes for families in the borough.

Family Wellbeing Practitioners work holistically with the whole family with our focus being children and young people who are at the heart of our practice. The role will require the practitioner to work within Merton’s Early Help Practice Model and be skilled to a high level in Systemic and Relationship Based Practice. Practitioners will also work to our service values which include strengths based perspectives, empowerment of families to make and sustain change as well as developing independence to underpin engagement and work with families.

As a Family Wellbeing Case Practitioner, you will be taking on the role of lead professional and engaging intensively with a caseload of up to 14 families in a diverse and vibrant borough with families who have complex needs. You will work on a voluntary and consent basis where a multi-agency, coordinated response is required to support in reducing risk factors that may lead to negative outcomes including developmental delays, non/low school attendance, statutory social care involvement and other vulnerabilities that could impact on families.

The role requires effective and direct close working with children, young people and adults (aged 0-25) and their families and key support networks. Providing advice, guidance and practical support that is impactful on a 3-6 months basis, underpinned by evidenced based practice and research.
