You will ideally be an experienced, autonomous nurse toprovide care for our patients; from initial history taking, clinicalassessment, diagnosis, treatment including prescribing where appropriate an devaluation of care for same-day care and minor illness/injuries. You will needto demonstrate critical thinking in the clinical decision-making process,prioritising and assessing the needs of the patient accordingly. Clinics arefully supported by our 11 GPs, and will include a mixture of e-consults, telephonetriage and same day face to face appointments.
You should be able to remain calm during busy periods, have great communication skills, IT knowledge and the ability to multi-task, as well as be confident to work on you own, but also aspart of a supportive and flexible team. Experience of Systm One and working inthe NHS would be an advantage.
Clinical Practice
Assess,diagnose, plan implement and evaluate treatment, interventions and care for patients presenting with an undifferentiated diagnosis.
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.
Assess,diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term condition.
Diagnose andmanage both acute and chronic conditions, integrating both drug andnon-drug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practiceprotocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over the countermedication, medication regimes, side effects and interactions.
Prioritisehealth problems and intervene appropriately to assist the patients in complex,urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promotehealthy lifestyles, and apply principles of self-care

Person Specification Qualifications- Registered General Nurse
- Relevant nurse practitioner qualification
- Recognised qualification to diploma level or above
- Minimum 2 years post-registration
- Management of minor illnesses or ailments
- Experience of primary care
- Experience of working in accordance with the NMC Code of Professional Conduct (2002) and The Scope of Professional Practice (1992)
- Experience of Microsoft Office software
- Experience of Systm One clinical system
- Experience of dealing with the public/patients
- Leadership and management experience
- Experience of working with teams
