Job Description

The Stroke Nurse Navigator will support patients throughout the healthcare process, from admission and diagnosis, through treatment, and to discharge and beyond. In addition to direct contact patient care, the Stroke Nurse Navigator will work closely with the Neuro Program Manager, Neuro Service Line Director, and the entire Stroke Team to assure provision of quality Stroke Patient care, identify opportunities for improvement, and support ongoing performance improvement initiatives.

This position requires in-depth knowledge of stroke diagnosis and treatment, as well as excellent communication and organization skills. Duties may include:

  • Assessing patients to assist in identification of patient specific needs and barriers, in order to provide assistance and overcome barriers to care.
  • Supporting bedside nurses in educating patients and caregivers about their diagnosis, treatments, side effects, risk factors and prevention.
  • Providing patients and their caregivers with sources for reliable information related to their condition and needs.
  • Promoting empowerment of the Stroke Patient in decision-making related to treatment.
  • Building and maintaining trust of patient and caregivers while providing emotional support related to stroke diagnosis and its impact on patient and family.
  • Acting as liaison between and among patient, family/caregivers, physicians, and other members of the healthcare team.
  • Collaborating with a multidisciplinary Stroke Team to coordinate patient care according to adopted guidelines.
  • Facilitating timely scheduling for tests and procedures.
  • Assuring that patient receives appropriate referrals and follow-up appointments.
  • Serving as a primary contact and advocate for patient and their caregivers.
  • Assisting in concurrent review of stroke patient data in order to assure quality care in real-time.
  • Collecting and compiling Stroke Patient satisfaction data and other duties related to performance improvement.
  • Providing required post-discharge outreach, such as 90-day mRS call.
  • Participating in stroke-related community outreach, including (but not limited to) Stroke Support Group and community events.
  • Implementing and evaluating quality assurance measures.
  • Education

    Bachelor's degree in Nursing is required.
    Master's degree is preferred.

    Experience

    3-5 years of work experience with stroke/neuro patients in an acute care environment is required.
    Familiarity with stroke diagnosis and treatment is required.

    License/Certificates

    RN License (Current VA License or a Multi-State Licensure privilege) is required.
    BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

    Qualifications

    Good communication skills, both verbal and written.
    Basic knowledge of spreadsheet (Excel) creation and use.
    Detail-oriented, efficient with timelines, good time management skills, ability to multitask.
