Job DescriptionThe Stroke Nurse Navigator will support patients throughout the healthcare process, from admission and diagnosis, through treatment, and to discharge and beyond. In addition to direct contact patient care, the Stroke Nurse Navigator will work closely with the Neuro Program Manager, Neuro Service Line Director, and the entire Stroke Team to assure provision of quality Stroke Patient care, identify opportunities for improvement, and support ongoing performance improvement initiatives.
This position requires in-depth knowledge of stroke diagnosis and treatment, as well as excellent communication and organization skills. Duties may include:
Bachelor's degree in Nursing is required.
Master's degree is preferred.
3-5 years of work experience with stroke/neuro patients in an acute care environment is required.
Familiarity with stroke diagnosis and treatment is required.
RN License (Current VA License or a Multi-State Licensure privilege) is required.
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Good communication skills, both verbal and written.
Basic knowledge of spreadsheet (Excel) creation and use.
Detail-oriented, efficient with timelines, good time management skills, ability to multitask.
