Registered Nurse (RN) – PACE Program (PRN)

Base pay range: $35.00/hr - $48.00/hr

About

The Role

We’re seeking a dedicated Registered Nurse (RN) to join our PACE interdisciplinary team on a PRN basis. In this flexible role, you’ll provide as‑needed support to the clinic and interdisciplinary team—delivering high‑quality, participant‑centered care that promotes health, independence, and dignity.

How You’ll Make a Difference
  • Perform daily nursing tasks and prepare the clinic for participant visits.
  • Communicate effectively with staff and keep the Clinic Manager informed of participant needs or concerns.
  • Respond to participant emergencies and provide timely clinical assessments.
  • Participate in care planning, implementation, and evaluation of individualized participant care.
  • Document all nursing care accurately in the electronic medical record.
  • Triage participants efficiently and support the interdisciplinary care process.
  • Administer medications and maintain proper medication inventory, including contingency and emergency supplies.
  • Perform phlebotomy procedures and other clinical tasks as needed.
  • Ensure the cleanliness and organization of the clinic and medication areas.
  • Provide wound care and maintain detailed wound documentation.
  • Schedule and coordinate medical appointments and assessments with providers.
  • Participate as a key member of the interdisciplinary team to deliver holistic care.
  • Oversee medication passes and monitor for medication error trends and quality improvement.
  • Perform other related duties as assigned.
What You Bring
  • Current Registered Nurse (RN) licensure in the Commonwealth of Virginia.
  • Current CPR certification.
  • Minimum of one year of experience working with the frail or elderly population.
  • Medical clearance for communicable diseases and up‑to‑date immunizations prior to participant contact.
  • Valid driver’s license, proof of insurance, and reliable transportation.
