AboutThe Role
We’re seeking a dedicated Registered Nurse (RN) to join our PACE interdisciplinary team on a PRN basis. In this flexible role, you’ll provide as‑needed support to the clinic and interdisciplinary team—delivering high‑quality, participant‑centered care that promotes health, independence, and dignity.
How You’ll Make a Difference- Perform daily nursing tasks and prepare the clinic for participant visits.
- Communicate effectively with staff and keep the Clinic Manager informed of participant needs or concerns.
- Respond to participant emergencies and provide timely clinical assessments.
- Participate in care planning, implementation, and evaluation of individualized participant care.
- Document all nursing care accurately in the electronic medical record.
- Triage participants efficiently and support the interdisciplinary care process.
- Administer medications and maintain proper medication inventory, including contingency and emergency supplies.
- Perform phlebotomy procedures and other clinical tasks as needed.
- Ensure the cleanliness and organization of the clinic and medication areas.
- Provide wound care and maintain detailed wound documentation.
- Schedule and coordinate medical appointments and assessments with providers.
- Participate as a key member of the interdisciplinary team to deliver holistic care.
- Oversee medication passes and monitor for medication error trends and quality improvement.
- Perform other related duties as assigned.
- Current Registered Nurse (RN) licensure in the Commonwealth of Virginia.
- Current CPR certification.
- Minimum of one year of experience working with the frail or elderly population.
- Medical clearance for communicable diseases and up‑to‑date immunizations prior to participant contact.
- Valid driver’s license, proof of insurance, and reliable transportation.
