  • Educate participants and families on health conditions, treatments, and preventive care
  • Participate in team meetings and contribute clinical expertise to participant care decisions
  • Maintain accurate medical records and documentation
  • Support quality improvement initiatives that enhance participant outcomes
  • Serve as a trusted clinical resource for staff and caregivers
What We’re Looking For

Required Qualifications
  • Graduate of an accredited nursing program
  • Current Registered Nurse (RN) licensure
  • Current Nurse Practitioner licensure
  • National Nurse Practitioner certification
  • Minimum two years of experience working with elderly or medically complex populations
  • Current CPR certification
  • Valid driver's license, reliable transportation, and proof of insurance
Preferred Qualifications
  • Geriatric Nurse Practitioner (GNP) certification with prescriptive authority
  • Two or more years of Nurse Practitioner experience in geriatrics
  • Experience in PACE, long-term care, home health, hospice, community-based care, or other interdisciplinary healthcare environments
Success in This Role

You’ll thrive if you:

  • Have a passion for caring for older adults
  • Value teamwork and collaboration
  • Possess strong clinical assessment skills
  • Are compassionate, patient-focused, and relationship-driven
  • Communicate effectively with patients, families, and care teams
  • Enjoy solving complex healthcare challenges and improving outcomes
What We Offer

We care for our team members the same way we care for our participants.
