Legacy Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to deliver hands-on nursing care, coordinate services, and guide patients and families through their hospice journey. Schedule includes Monday–Friday, 8:00 am–4:30 pm with rotating call, serving Clarke, Frederick, Shenandoah and Warren County.
You will collaborate with our interdisciplinary team, empower patients and families through education, and advocate for personalized care plans.
