Community Health Resources is hiring a Community Support Worker for the Community Support for Families program in Willimantic, CT. This full-time, 40 hours/week role provides coordination of care for families referred from DCF's Differential Response System. Workers and Parent Navigators assist families through a child and family team process to develop care plans based on strengths and needs. The team collaborates to meet the needs of families with basic needs, behavioral health, substance use, and connection to community services.

Community Support for Families provides coordination of care for children and adults referred from DCF's Differential Response System. Community Support Workers and Parent Navigators assist the family through a child and family team process that develops care plans based on functional strengths and needs. The workers and the community work as a team to meet the needs of families who may present with a variety of basic needs, behavioral health, substance abuse and/or linkage to community services.
Duties & Responsibilities- Assess overall family needs including basic needs, therapeutic, medical, and other needs
- Provide support and crisis management related to family needs
- Link families with needed community support services, programs, and agencies; advocate for clients’ rights and access to services
- Serve as liaison with DCF, hospitals, other community-based services, family members, and significant others
- Process appropriate requests for flex funding
- Collaborate with Parent Navigators
- Additional duties as assigned
- Education: A bachelor’s degree in a Human Services related field is preferred, an associate’s degree plus two years’ experience in a related field in lieu of bachelor's, or high school diploma with a minimum of 4 years’ experience in related field will be considered
- Experience: Minimum of 2 years' experience in a mental health related setting
- Licesure/Certification/Registration: Valid driver’s license
- Other Qualifications: Bilingual in Spanish and English preferred
Competitive compensation based on experience. Minimum $20.00/hour, with additional consideration for experience and internal equity; rates vary by program/location and may include sign-on incentives. For part-time positions, pay is pro-rated based on hours worked.
