At Senior Solutions (Area Agency on Aging in southeastern Vermont), we are seeking a Case Manager to work with older adults and/or people with disabilities in Windsor, Vermont, and the surrounding areas using a person‑centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required.

• Bachelor’s degree in human services and prior case management experience preferred.
• A combination of relevant education and experience may be substituted for a degree.
• Must have reliable transportation and a valid driver’s license.

• Collaborating with other community partners to create a person‑centered plan of care.
• Monitoring and reassessing program service plans as required.
• Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
• Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
• Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
