Support the Sisters of the Holy Names of Jesus and Mary with essential healthcare coordination in Windsor, Ontario. This part-time RN role focuses on providing compassionate care and medical advocacy.
As a Healthcare Coordinator, you'll offer individual support to the Sisters, assisting with health-related services and their preference for independence. Your responsibilities will include care coordination, medical transportation, and transitioning Sisters to various care levels. Collaborate with the Director of Healthcare Services while ensuring the Sisters understand their health needs and receive optimal care.
Key Responsibilities:
• Advise Sisters on healthcare and services needed
• Coordinate care plans and monitor progress
• Provide transportation to medical appointments
• Document clinical information in the SNJM database
• Assist with educating Sisters about wellness topics
Requirements:
• RN with 8-10 years of geriatric experience
• BScN preferred; registered with CNO
• Experience in case management preferred
• Valid driver’s license and good driving record
• Strong organizational and communication skills
