Join Windsor Regional Hospital as a Social Worker, focusing on the psychosocial support of patients within the Medicine/Surgery and Women & Children's departments. Collaborate closely with healthcare professionals to enhance patient care.
In this role, you will fulfill the psychosocial needs of patients from admission through discharge and facilitate effective rehabilitation and community reintegration. Your knowledge of available community services, as well as strong interpersonal and communication skills, will contribute significantly to the multidisciplinary team’s success.
Key Responsibilities:
• Assess psychosocial needs of patients for effective care
• Create tailored rehabilitation and discharge plans
• Connect patients with community service resources
• Collaborate with healthcare professionals to improve care
• Uphold Occupational Health and Safety standards
Requirements:
• 1-2 years discharge planning experience in acute care
• Required Bachelor's degree in Social Work
• Must be registered with Ontario College of Social Workers
• Excellent communication and interpersonal skills
• Commitment to quality assurance and departmental standards
