The CMHA-WECB is seeking a dedicated Mental Health Navigator to work collaboratively with the Clinical Services manager and Community Partner leadership. In this temporary position, you will conduct mental health and addiction assessments while providing essential counseling for clients. Your work will help to develop care plans and outcomes tailored to individual needs, ensuring seamless service delivery.
Key Responsibilities:
• Conduct and complete mental health assessments for clients
• Provide supportive counseling until program entry
• Collaborate with community agencies for support navigation
• Create and implement care plans with client input
• Facilitate seamless transfer of care during transitions
Requirements:
• University degree in relevant field (e.g., BSW, BScN)
• Registered professional member in good standing
• Two years relevant mental health clinical experience
• Knowledge of mental illness and addiction services
• Valid Ontario Driver’s License and vehicle access
