CORE RESPONSIBILITIES OF THE POSITION  The Nurse Practitioner (NP) provides autonomous comprehensive primary care to individuals and families across their lifespan. The focus is on health equity, chronic disease management, prevention and care for the underserved populations. Working within an interprofessional team, the NP delivers patient-centered care aligned with the values of community health and social determinants of health.
Perform full scope assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) as well as the Medical Directives of weCHC
Carry own roster of clients/patients as the primary care provider
Order and interpret diagnostic tests (labs, imaging)
Prescribe medications in accordance with provincial guidelines
Maintain accurate, timely documentation in Electronic Medical Record (PS Suites)
Deliver trauma informed, culturally safe care
Make referrals to support client needs and goals
Work closely with physicians, nurses, social workers, dietitians and other team members
Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED   Minimum 1-2 years’ experience as an Extended Class Nurse Practitioner working with vulnerable clients
Experience working with mental health and addictions is an asset
Experience in a community health setting or combination of community health and hospital or public health settings
Strong attention to patient-centred care and establishing therapeutic relationships required
Demonstrated ability to work in an inter-professional team environment required
Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
Proficient in the use of computers and various software applications
Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County and the province, as required
Bilingual (English/French/Spanish) language skills an asset.
EDUCATION, TRAINING AND CERTIFICATE REQUIREMENTS   Undergraduate and/or Graduate degree in Nursing from a recognized University
Qualified to provide care to clients across their lifespan
Member in good standing in the Extended Class with the College of Nurses of Ontario
Member in good standing with the Nurse Practitioner’s Association of Ontario (NPAO) and/or Registered Nurses Association of Ontario (RNAO)
Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment.
