Primary Health Care Nurse Practitioner Position

As a full-time Nurse Practitioner at weCHC, you will leverage your skills to deliver patient-centered care within an interprofessional team. Your responsibilities include performing assessments, ordering diagnostic tests, and prescribing medications in accordance with provincial guidelines. This role emphasizes supporting underserved populations and meeting their unique healthcare needs through compassionate care.

Key Responsibilities:

• Conduct full assessments, diagnoses, and treatments

• Manage a personalized client roster as primary care provider

• Order, interpret, and analyze diagnostic tests

• Prescribe medications following guidelines

• Maintain accurate documentation in electronic records

Requirements:

• 1-2 years as an Extended Class Nurse Practitioner

• Experience with mental health/addictions is an asset

• Strong focus on patient-centered care

• Excellent communication and teamwork skills

• Valid Class “G” Driver’s License required
