In this essential role, the Registered Practical Nurse will report to the Nurse Manager and lead nursing practices focused on residents' needs. Collaborating with the interdisciplinary team, you’ll engage in assessments, medication management, and family communication to ensure optimal care plans and outcomes.
Key Responsibilities:
• Deliver safe nursing care centred on resident preferences
• Conduct assessments and modify care plans based on needs
• Document health changes and communicate effectively with families
• Maintain CNO standards with thorough documentation
• Guide and support PSWs while promoting teamwork
Requirements:
• Registered Practical Nurse with Ontario's CNO
• CPR/BLS certification is current
• Strong communication and collaboration skills
• Experience with interRAI documentation processes
• Committed to respectful, person-centered care
