Join Ontario Health atHome as a Palliative Care Nurse Practitioner, where you will provide essential patient-centered services across the Erie St. Clair region. This role emphasizes teamwork and independence within diverse care environments.
Your mission is to deliver comprehensive assessments and manage palliative symptoms while collaborating with a multidisciplinary team. You will apply your specialized knowledge to enhance patient outcomes and ensure quality care as part of the Palliative Care Consultation Team. Creativity in problem-solving and time management will be key.
Key Responsibilities:
• Assess and manage complex pain and symptom issues
• Collaborate with other health professionals for patient care
• Facilitate referrals to community services
• Conduct home visits and offer hospital support
• Ensure adherence to professional standards and guidelines
Requirements:
• Registered Nurse Practitioner with proper licensing
• Solid understanding of community health resources
• Specialized training in palliative care
• Access to a vehicle for site travel
• Proficient in computer use for documentation
