Join Windsor Essex Community Health Centre as a Clinical Care Coordinator/Registered Nurse in Windsor, ON. This full-time role focuses on advocacy and care coordination for the Refugee Health program.
WeCHC is seeking a Clinical Care Coordinator, Registered Nurse with 3-5 years of nursing experience. In this position, you'll support clients with complex health needs while collaborating with health professionals and community organizations to enhance care pathways. Your skills in program development and sensitivity in dealing with vulnerable populations will be key in this rewarding role.
Key Responsibilities:
• Serve as a resource for refugees and community members
• Build partnerships with internal and external organizations
• Conduct health assessments and education for clients
• Field program inquiries and coordinate client scheduling
• Document care plans and participate in case reviews
Requirements:
• 3-5 years nursing experience in community settings
• Knowledge of health care systems and navigation
• Master of Science in Nursing (MScN)
• Valid registration with College of Nurses of Ontario
• Clean driver’s abstract and reliable transportation
