Support pediatric patients in school settings with Para Med as a Registered Practical Nurse, using assessment and care planning skills. Weekday school hours apply during the school year.
This School Nurse and Community Visiting Nurse role serves pediatric patients with differing care complexities. During school breaks, community visits support patients of all ages work includes assessment, care plan monitoring, collaborative rapport building, and professional record maintenance.
Key Responsibilities:- Assess individualized patient needs using appropriate tools and techniques
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative patient engagement
- Maintain and update professional patient records
- Provide community home visits during school breaks
- Current CNO registration as a Registered Practical Nurse
- Valid driver’s license and access to a vehicle
- Pediatric care experience considered an asset
- Community care experience considered an asset
- Willingness to provide required on-call coverage
