Assess pediatric patients in Amherstburg schools while providing community nursing care, individualized plans, and trusted patient relationships. This is a part time position.
As a School Nurse and Community Visiting Nurse with Para Med, you will care for pediatric patients in school settings. During school breaks, you will provide home visits to patients of all ages with differing care complexity. The work includes assessment, care plan monitoring, collaborative engagement, and professional record maintenance.
Key Responsibilities:- Assess individualized patient needs using appropriate assessment techniques
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative patient engagement
- Maintain and update professional patient records
- Provide home visits during school breaks
- Current CNO registration as a Registered Practical Nurse
- Valid driver’s license and access to a vehicle
- Pediatric care experience considered an asset
- Community care experience considered an asset
- Reliability, flexibility, and willingness to travel
