You will function as a dedicated advocate, conducting needs assessments and offering hands-on assistance on vital resources. A focus on the Social Determinants of Health framework will shape your interactions, helping clients overcome challenges related to income and housing. You will also ensure accurate documentation and collaborate with peer workers to enhance program delivery.
Key Responsibilities:
• Carry out comprehensive assessments for client care
• Provide empathetic, client-centered support based on SDOH
• Help secure basic needs through resource coordination
• Facilitate government application processes for clients
• Maintain case documentation and assist in program oversight
Requirements:
• Relevant diploma or degree in social sciences
• Demonstrated experience with diverse populations
• Proficient communication skills for client engagement
• Experienced in Microsoft Office and case management systems
• Possess a valid driver’s license for field outreach
