Position: Social Worker for Complex Needs
Join Hôtel Dieu Grace Healthcare as a Mental Health Social Worker I, focusing on complex special needs in Windsor, Ontario. Collaborate with a dedicated team to provide impactful support for youth and their families.

As a key member of the inter-professional team, you’ll leverage 2-4 years of relevant social work experience to coordinate service applications and facilitate referrals. You will play a crucial role in creating transition plans and ensuring the welfare of children with complex mental health needs. Your adherence to safety standards will promote a secure environment for clients and staff.

Key Responsibilities:

• Facilitate community case resolution and meetings

• Coordinate placements and respite support

• Manage budget preparation and application processes

• Prepare reports according to established guidelines

• Attend and lead Plan of Care meetings

Requirements:

• BSW degree is a must

• Certification from Ontario College of Social Workers required

• 2-4 years experience with children's services preferred

• Understanding of the social services system is beneficial

• Proficiency in French is an asset
