The Welcome Centre Shelter seeks a full-time Case Manager for Complex Needs. This role requires expertise in crisis support and case coordination for high-need populations.
In this position, you will collaborate with individuals facing significant challenges, including mental health issues and domestic violence.
Your role will be crucial in providing individualized support, conducting assessments, and navigating complex social service systems. Strong organizational and communication skills are essential to success in this emotional and rewarding environment.
Key Responsibilities:
• Facilitate access to necessary social services
• Develop comprehensive Individualized Service Plans
• Assess client needs and coordinate appropriate interventions
• Engage with community partners for enhanced client support
• Maintain up-to-date client documentation per agency standards
Requirements:
• Required university degree in social or health sciences
• At least 1 year of case management experience
• Valid driver's license and reliable vehicle for travel
• First Aid/CPR Certification before starting
• Familiarity with local low-income support resources
