We are seeking an experienced RN to serve as a Clinical Case Manager in our home health agency. The ideal candidate will have a positive attitude, proficiency with OASIS and a home care background.

• Provide clinical case management, including OASIS completion, care coordination, and client advocacy.
• Collaborate with interdisciplinary team to develop and execute individualized care plans.
• Document and maintain accurate clinical records in compliance with agency and regulatory standards.

• RN required with 2 years of nursing experience (home health preferred).
• 1 year of home care experience and proficiency in OASIS completion.
• Bachelor’s degree preferred.
• English required.
• Must hold a valid RN license.

• Monday to Friday, Weekend on-call rotation.
• Two weeks paid time off.
