Weekend Registered Nurse RN Home Health

Elara Caring invites an experienced Registered Nurse to join our elite team providing home health care through a weekend schedule (Friday, Saturday, Sunday or Saturday, Sunday, Monday). This role supports the mission to deliver right care at the right time, in the right place by ensuring patients receive individualized, compassionate, and high-quality nursing services in their homes.

• Ensure care activities align with Elara Caring’s vision and home health team leadership.
• Maintain patient and staff privacy pursuant to HIPAA.
• Assess assigned case load of home‑health patients and families to identify physical, psychosocial, and environmental needs.
• Assume primary responsibility for caseload, developing, implementing, and evaluating patient plans of care.
• Conduct initial and follow‑up assessments, revising plans as needed.
• Communicate significant findings and changes in patient condition to the Clinical Supervisor, physician, and other team members.

• Current, unrestricted state RN license.
• Minimum 1 year of clinical nursing experience.
• Willingness and ability to travel within the branch/office coverage area; dependable vehicle, valid driver’s license, and auto insurance.
• Associate’s or Bachelor’s degree in Nursing preferred.
• Previous home health or hospice experience preferred.
• Experience with Home Care Home Base preferred.
