. We are seeking an RN to deliver dialysis care in skilled nursing facilities (SNF). You’ll provide on‑site treatment to patients with end‑stage renal disease, offering high‑quality, compassionate care directly in the facility setting.
- Provide dialysis treatment to patients at SNF partner sites
- Monitor patient vitals and assess treatment response
- Collaborate with interdisciplinary teams to coordinate care
- Educate patients and families on treatment and care plans
- Maintain safety, quality, and regulatory standards
- Current RN license in the state of practice
- ADN required; BSN preferred
- Minimum of 1 year of Chronic or Acute Care Dialysis Experience required
- ICU, CCU, ER, or Med/Surg experience a plus
- CNN or CDN certification preferred
- Medical, dental, vision, and 401(k) match

This position will be open for a minimum of three days.
